❊ Menu selection
(Regular or Vegetarian if applicable to that menu)
❊ Gift Card Number

Gift card funds can also be used toward drinks/add on’s during your visit.
If you’d like to ONLY use your gift certificate for drinks/add-on’s at the time of your visit then
you can go ahead and book online to pay for dinner.
